Please download and complete the Liability Waivers and submit them along with the applicable registration fees to the Club at the Parent/Player meeting. This meeting will be scheduled for early January, 2010.

Please note the addition of the new Concussion Information form that has been added this year.

The Registration Fee for the 5/6 and 7/8 Teams remain at $150 per player.
The 3/4 Team Registration Fee is $130 per player.
